Vaccination Check List

The start of a new school year is an ideal time to schedule a yearly physical for your children and make sure all their vaccinations are up-to-date. If your child is not up-to-date on their vaccinations, don’t worry. Most shots can be given at a later age.

This chart is provided only as a sample for children ages four and older. In certain high-risk groups, he or she may require additional vaccines. For example, with asthma or other lung diseases, sickle cell anemia, diabetes or another chronic medical condition, your child should probably be given a flu vaccination. Be sure to see your pediatrician or family physician to determine the specific vaccinations that your child needs.

Ages 4 – 6

DTaP (Diphtheria,

Tetanus, Pertussis)

Polio

MMR (Measles, Mumps,

Rubella)

Varicella Booster

Ages 11 – 12

DTaP (Diphtheria,

Tetanus, Pertussis)

HPV (Human Palillomaviros,

for females)

MCV4 (Meningococcal)
