Your Kids Best Shot
The start of a new school year is a good time to schedule a yearly physical for your children and make sure all their vaccinations are up-to-date. This chart is provided only as a sample for children ages four and older. In certain high-risk groups, he or she may require additional vaccines. Be sure to see your pediatrician or family physician to determine the specific vaccinations that your child needs.
Ages 4 – 6
__ DTaP (Diphtheria, Tetanus, Pertussis)
__ Polio
__ MMR (Measles, Mumps, Rubella)
__ Varicella Booster
Ages 11 – 12
__ DTaP (Diphtheria, Tetanus, Pertussis)
__ HPV (Human Palillomaviros, for females)
__ MCV4 ( Meningococcal)
Up to Age 18
The following can be administered up to age 18 if the child has not
received the vaccine:
__ Hepatitis B
__ Polio
__ MMR (Measles, Mumps, Rubella)
__ Varicella Booster
